Testing at another rink?
If you plan to test away from Washington Park Ice Arena, you need to contact the test chair and/or treasurer to make sure you have the correct information. You will need a "permission to test" form signed by the Test Chair (Barb Bruemmer) or the Treasurer (Kristi Heritage )! You need a new form signed EVERY time you test! Not obtaining a permission to test could make the test invalid!

Steps to obtain a Permission to Test form
1. Download the fillable form and fill it out.
2. Email the form to the test chair or treasurer for
signature or print and bring to the rink for signature.
You can also mail it to the PO Box.
(Form can not be returned by email).
